What Is Olanzapine?
Olanzapine belongs to the family of atypical, or second-generation, antipsychotics. It is used mainly in the treatment of schizophrenia and bipolar disorder.
Compared with classic antipsychotics, this generation of molecules generally causes fewer motor disorders, known as extrapyramidal effects. In exchange, it carries a higher risk of metabolic effects, including weight gain and disturbances in blood sugar and lipid levels.
Origin and Discovery: Building on Clozapine's Legacy
Olanzapine is a synthetic molecule developed by the pharmaceutical company Eli Lilly. It was approved in 1996.
It was designed in the wake of clozapine, the first atypical antipsychotic, which was highly effective but limited by a hematological risk. The goal was to match that efficacy while achieving a more favorable tolerability profile.
Olanzapine quickly became one of the most widely prescribed antipsychotics.
Chemical Structure and Industrial Synthesis
Olanzapine belongs to the thienobenzodiazepine chemical family. Its structure, closely related to that of clozapine, combines a benzodiazepine core with a thiophene ring.
It is produced through industrial chemical synthesis. An orodispersible form, a tablet that dissolves on the tongue, as well as an injectable form, were developed to make administration easier.
Mechanism of Action: A Multi-Receptor Profile
Olanzapine acts as an antagonist at numerous brain receptors. This 'multi-receptor' profile accounts for both its efficacy and its side effects.
It blocks serotonin (5-HT2A) and dopamine (D2) receptors, which explains its antipsychotic effect. It also acts on histamine (H1) receptors, responsible for sedation and weight gain, as well as on muscarinic and adrenergic (α1) receptors, which underlie atropine-like effects and hypotension.
The high ratio between 5-HT2A and D2 receptor blockade accounts for the molecule's 'atypical' character, with fewer motor effects than classic antipsychotics.
Therapeutic Indications and Monitoring
The main indications for olanzapine are schizophrenia, both acute episodes and relapse prevention, as well as moderate to severe manic episodes associated with bipolar disorder.
It is also used to prevent recurrence in bipolar patients who respond to treatment.
Because of the associated metabolic risk, weight, blood glucose, and lipid levels should be monitored regularly in treated patients.
